Diagnosing Low Testosterone

Hormonal changes naturally occur as men age. Understanding and testing for low testosterone is the first step toward optimizing hormones to maintain your vitality and optimal functioning. Through lab testing, we assess three types of testosterone levels in your body: total testosterone, free testosterone, and bioavailable testosterone.

Total testosterone, as the name implies, shows the total testosterone level in your body. Of this testosterone, the majority is bound to sex hormone-binding globulin (SHBG). Testosterone bound to SHBG is inactive, and is not considered bioavailable.

Free testosterone in the body is testosterone that is not bound to SHBG, and represents the active portion of total testosterone levels.

Bioavailable testosterone refers to the testosterone in the body that is active and can be used up by your body. Historically, we thought only free testosterone was bioavailable, but research shows that testosterone bound to serum albumin is also bioavailable - and the levels of testosterone bound to serum albumin is not accounted for in the free testosterone lab level.

It’s important to test for all three of these testosterone levels, as some men can have normal levels of total testosterone but low levels of free and bioavailable testosterone, which can result in experiencing symptoms of low testosterone.
